MOW Enrollment

Meals on Wheels program provides a home-delivered meals to individuals who are eligible. Please review basic eligibility and enrollment process below.

Program Eligibility 

Meals on Wheels program is for individuals who are:

  • 60 years of age or older; AND
  • Homebound - unable to leave their home without someone's assistance; AND
  • Physically or mentally frail; AND
  • Not capable of preparing simple meals without someone's assistance; AND
  • Not living with someone's capable of preparing simple meals; AND
  • Without adequate family or paid support for meals
Program Prioritization 

All Meals on Wheels Yuba & Sutter Counties program participants are evaluated and prioritized based on their need for meals. Because of high interest in the program, we established a prioritization process to assure individuals struggling with food insecurity, recovery from hospitalization, social isolation, nutrition risks, multiple chronic diseases, risks for falls and other program-related issues have higher priority for receiving home-delivered meals. 

Enrollment Process

Individuals interested in home-delivered meals must agree to our staff in-home visit and complete intake process to assure they meet eligibility criteria and complete prioritization process. Once the initial eligibility and prioritization process is completed individuals may be placed on the program accordingly. All program participants are reevaluated and assessed on a quarterly basis. Staff will alternated quarterly reassessments between in-home and phone appointments. At least two assessments per year must be in-home visits.
